How To Complete the Housing Application Form
We want to get a clear picture of the way you are living to register your needs for re-housing. All the information you give us is treated confidentially.
The following tips will help you fill in the form and make sure you have not forgotten to tell us something important that may affect the number of points we give you.
Please make sure that the answers you give are true. You may be refused housing if your give false information.
Section 1 - About you (and joint applicant)
Fill in your name, address and phone number, if you have one, in the white boxes.
If you are applying jointly with someone else, fill in their name, address and phone number too.
Section 2 - About your Household
First, write your name again and the name of your joint applicant if you are applying with another person. Then fill in the names of everyone that will be living in your home with you. Give each person's date of birth (including your own), whether they are male or female, their relationship to you (for example partner, son, daughter) and if they are living with you now.
Then tick the box below to show if anyone included in your application is pregnant. If yes, tell us the name of the person who is pregnant and the expected due date.
Tell us about any pets you have. By law, we must also ask you to tell us if you or anyone on your application is related to a member of staff or a committee member of any Registered Social Landlords (formerly Housing Association).
Section 3 - About your present home
Tick the box which best describes where your are living now. If the type of property you are living in is not shown, please tell us in the space marked 'Other'.
Answer each of the other questions about your home by ticking 'Yes' or 'No' or by giving the information that is asked for, including telling us how long you have lived in your current home. You need to tell us in this section if you share or lack any facilities, and about the state of the property if it is in poor repair.
Section 4 - About where you lived before
Fill in this section to show where you have lived during the last three years and how long you have lived in Shepway;. You must tell us if you have had a Council or Registered Social Landlords (formerly Housing Association) tenancy before; and if you lost a tenancy for any reason. The dates are important if you have lived in more than one property during this time.
Section 5 - About your health
We need to know if you or any of the people listed on your application form to be re-housed with you have any health problem or special needs. Please tick the 'Yes' or 'No' boxes for each question on the form.
If you tick 'Yes', please tell us their name(s) and explain their circumstances. We will then give you a medical form which you need to fill in and send back to us. Our independent medical advisor will tell us how many points we need to give you in this section.
Section 6 - About your Income
We need to know about your income because some Registered Social Landlords (formerly Housing Association)s will take this information into account when assessing your housing need for their properties. We also need this information if you are an owner-occupier applying for rented accommodation.
Section 7 -Why do you need housing?
Tick all the boxes that apply to you and underline the most important reason. You can give us any other information you think would be useful.
Section 8 - Where would you like to live?
Tick the boxes on your application form telling us which areas you want to choose. The more areas that you choose, the greater your chance of being offered accommodation.
You need to tick any of the boxes telling us if you want housing for older people, shared ownership, or a housing co-perative.
Tell us if you want to be considered for a Registered Social Landlords (formerly Housing Association) property.
Section 9 - Equal opportunities
This section helps us make sure every applicant gets an equal opportunity on the Shepway waiting list. You do not have to fill it in but it does give us useful information.
The Council believes that everyone should be treated equally and fairly whatever their race, colour, nationality, ethnic origin, religion, age, disability, sex, sexuality or marital status.
Declaration
You must sign and date this form telling us that the information you have filled in is, to the best of your knowledge, true. Your signature and the date are important.
